  Collectively, they provide physical, emotional and spiritual support to the patient and family. At hospice IE and OC  we listen to your wishes and work with the patient and family to develop a care plan individualized to your needs and preferences.The Hospice interdisciplinary team creates an individualized plan focused on monitoring pain, managing symptoms, addressing nutritional needs, watching for emotional issues, and offering support.

Medical Directors

Physicians who collaborate with the hospice team and the patient’s primary care physicians to oversee the plan of care.

Registered Nurses

Specialists in hospice care and pain management who are available 24/7 to assist with any concerns that may arise.

Home and Health Aides

Specially trained to assist patients with activities of daily living and personal care (bathing,shaving,etc).

Medical Social Worker

Patient and family advocates who assist with placement, funeral arrangements, financial matters, and community resources.

Chaplains

Spiritual Counselors who provide non-denominational faith support and bereavement support to patients, families, and staff.
